The problem

Your medical history didn’t come with the movers.

Every doctor you’ve seen keeps their own piece of it. Change cities or insurance and the new office hands you a clipboard: list your medications, your surgeries, your allergies, the name of that specialist from three years ago.

You don’t have to move to lose your history. Switch practices across town and the new office starts you from the clipboard too.

Getting the real records means calling each old office, filling in their forms and waiting. In the middle of a move, that’s one more job.

7

physicians a year, across four practices, for a typical Medicare patient. Not one holds the whole story.

How it works

Three steps, and far fewer calls to old offices

  1. Start your trial

Say who the profile is for, and your new address.

  2. Triangle does the asking, or you upload

Ask Triangle to request your records from your old doctors through the national networks, Carequality and TEFCA (you verify it’s you, sign one release, and meet a clinician who signs the request with you), instead of you calling each old office. Or upload anything you’re holding: PDFs, photos of paper records, discs. Or both.

  3. Unpack and share

Your records arrive in one history, read in plain language, with what may still be missing. Share it with your new doctor and choose who can see it.

Questions for the new city
  • “Write a short summary of my asthma and blood pressure history for my first visit with my new doctor.”
  • “List my medications with doses and last fill dates for the new-patient form.”
  • “What did my cardiologist in Chicago note at my last visit?”

What your assistant reads is handled under its own privacy policy, which Triangle doesn’t control; disconnect it at any time in Triangle.

Questions

Before you move

What does it cost?

A 7-day free trial, then $9 a month. The trial needs a card. Cancel before day 7 and you’re not charged; after that, cancel any month, and your records stay yours.

Membership covers Triangle requesting your records from your old doctors and hospitals and keeping them updated as new records arrive, chat anytime with an assistant that answers from your own records, a review of your medical history by a licensed clinician, uploading anything else you have, and connecting your wearables.

Will Triangle reach my old doctors?

Triangle requests records through the national health-record networks, Carequality and TEFCA, which connect many hospitals and clinics. Not every office can be reached that way. Carryall shows what may still be missing and helps you request it, and you can upload anything you have, including photos of paper records and discs.

Does Carryall help me find a new doctor or choose insurance?

No. Carryall doesn’t recommend doctors or insurance plans. It helps you bring your history with you, wherever you go next.

Can Carryall refill a prescription, diagnose or prescribe?

No. Triangle does not diagnose, treat or prescribe, and it can’t refill a prescription. Your new doctor handles your care. Carryall helps you arrive prepared.
